What happens during
your in-office visit
Intake & Feeding History
We begin every visit by getting to know you and your baby — your birth story, your feeding history, your challenges, and your goals. This is not a quick intake form. It is a real conversation, because context matters. Nothing you share will be met with judgment.
Physical Assessment
We assess your baby's oral anatomy, feeding mechanics, and any physical factors that may be affecting latch or milk transfer. We also evaluate your breast health, nipple condition, and any concerns you have about your milk production or supply.
Live Feeding Observation
We observe a complete feeding in real time — watching your baby's latch, positioning, jaw movement, and swallowing. We provide hands-on guidance as needed and can make adjustments during the feeding so you can feel the difference immediately.
Weighted Feed
Using our medical-grade infant scale, we weigh your baby before and after a full nursing session to calculate exactly how much milk was transferred. This is one of the most valuable pieces of information we can gather — it removes the guesswork about whether your baby is getting enough to eat.
Pump Evaluation & Flange Fitting
If pumping is part of your feeding plan, we evaluate your pump, review your settings, and physically fit your flanges for the correct size. Incorrect flange sizing is one of the most common — and most fixable — causes of pumping pain and low output.
Personalized Care Plan
Every visit ends with a written, personalized care plan. Not a generic handout — a document specific to you, your baby, and your feeding goals with clear next steps, follow-up instructions, and contact information. You leave knowing exactly what to do next.

Flange Sizing & Fitting
Physical flange fit assessment — measuring your nipple diameter and evaluating how your current flanges fit during a live pumping session. One of the most impactful interventions we offer.
Blocked Duct & Breast Assessment
Physical assessment for blocked ducts, mastitis concerns, engorgement, and other breast health issues. Including access to in-office therapeutic ultrasound for clogged duct treatment.
Oral Anatomy Assessment
Assessment of your baby's tongue mobility, lip tie, and oral mechanics that may be affecting latch or milk transfer — with discussion of next steps if intervention is recommended.

While we love virtual visits for many situations, certain concerns are best addressed in person.
Weight gain concernsIf your baby has not regained birth weight or your provider has expressed concerns about weight, a weighted feed gives us objective data.
Significant nipple painSevere latch pain, cracking, or nipple damage often requires hands-on assessment and real-time positioning correction to resolve.
Tongue or lip tie concernsOral anatomy assessment is best done in person so we can fully evaluate your baby's latch mechanics and tongue mobility.
Pumping pain or very low outputPhysical flange fitting and a live pumping session observation give us information that is difficult to gather virtually.
Suspected blocked duct or mastitisPhysical breast assessment is essential for evaluating severity and determining the right treatment approach.

to your appointment
There is no special preparation required before an in-office lactation visit. Come as you are — tired, uncertain, relieved, or all of the above. This is a safe space and whatever you are carrying right now is welcome here.
A few things that are helpful to bring so we can make the most of your time together:
Partners and support people are always welcome. Having your partner in the room often makes a significant difference in feeding outcomes, because they can observe, learn, and support you when you are home and we are not there.
If you are not sure what to bring or have specific questions about your visit, call or text us anytime before you come in.

- Your breast pump if you are using one
- All pump parts including flanges you currently use
- Any bottles or nipple shields you use
- Your partner or support person if possible
- Your questions — all of them, no matter how small
- Any nipple creams or products you currently use
- A change of clothes for you — things can get milky
- A blanket or swaddle your baby is comfortable in

Where exactly is your office?

How long does an in-office visit take?
Most in-office consultations run 60 to 90 minutes. Initial visits typically take longer as we go through a thorough intake and assessment. Follow-up visits may be shorter depending on what we need to address. We do not rush — you have our full, undivided attention for the duration of your appointment.
Should my baby be hungry when we arrive?
It is helpful if your baby has not just had a full feeding immediately before the appointment, since we will want to observe a feeding and do a weighted feed. However, please do not let your baby go longer than they normally would just to time it for us. A mildly hungry baby is ideal — a frantically hungry baby makes things harder for everyone. Do what is best for your baby.
Can my partner come with me?
Yes — and we encourage it. Partners who observe a lactation consultation tend to become significantly more effective support people at home. They can ask their own questions, learn the correct hold and positioning, and understand what is actually happening with feeding. Bring them.
Is the in-office visit covered by insurance?
Yes — for most families. In-office lactation consultations are covered under the ACA lactation benefit that requires most insurance plans to provide breastfeeding support. We accept IEHP, Kaiser, Medi-Cal, UnitedHealthcare, Health Net, Blue Shield Promise, Community Health Group, Cigna, and most PPO and HMO plans. Verify your benefits here before booking.
